Supported Case Management

Case Management is a process by which goals are determined for an individual, in collaboration with the individual, and a plan is put in place to achieve the necessary goals. These goals can relate to all facets of a person's life, such as health care, resolution of legal issues, counselling, finance issues, etc. These goals are listed and a plan for how these goals will be achieved is completed. This is called a Case Plan. A Case Plan often involves the combination, coordination and collaboration of different services, to ensure that the individual goals are met in a timely fashion.
Healing Hearts staff have extensive experience in creating, facilitating and working through Case Plans, and networking with different agencies, stakeholders (the people who have a connection to, or "stake" in the goals being achieved) and partners. As such, Healing Hearts staff are able to offer this service to individuals and families to support a person's journey towards independence and/or management of mental health and/or alcohol and other drugs issues.
